Naloxone, which reverses the effects of opioids, was synthesized in 1960, though the hunt for opioid antagonists began a half-century earlier. The history of this quest reveals how cultural and medical attitudes toward opioids have been marked by a polarization of discourse that belies a keen ambivalence. From 1915 to 1960, researchers were stymied in seeking a “pure” antidote to opioids, discovering instead numerous opioid molecules of mixed or paradoxical properties. At the same time, the quest for a dominant explanatory (...) and therapeutic model for addiction was likewise unsettled. After naloxone’s discovery, new dichotomizing language arose in the “War on Drugs,” in increasingly divergent views between addiction medicine and palliative care, and in public debates about layperson naloxone access. Naloxone, one of the emblematic drugs of our time, highlights the ambivalence latent in public and biomedical discussions of opioids as agents of risk and relief. (shrink)

Five of the six contributions to this varied and valuable collection of essays originated as papers delivered at the 1999 Roman Archaeology Conference in Durham, England. The sixth and longest (...) piece, by Benet Salway, was then added, together with an introduction by one co-editor (Adams) and an afterword by the [End Page 529] other (Laurence). Notes follow each contribution, with a consolidated bibliography and index at the end. Although in black and white only, the range of photos, figures, and maps is appropriately extensive.The volume stems from due recognition that geographical knowledge and travel in the Roman world are topics at last attracting the serious attention they deserve. Between them, the contributions amply demonstrate how even some of the aspects most fundamental to our understanding stir deep controversy and how incomplete the exploitation of much of the diverse source material has been to date. Happily, the emergence of exciting new discoveries is prompting fresh examination of testimony long known and neglected.Useful, though limited in scope, is Anne Kolb's "Transport and Communication in the Roman State: The cursus publicus" (95-105), which mainly confines itself to identifying those categories of imperial officials and other individuals entitled to use the service (more properly referred to as vehiculatio during the Principate). By contrast, the support Kolb offers for the doubtful claim that transport of freight—other than baggage accompanying officials—was a "relatively unimportant" aspect of the service, even in the Late Empire (102), remains inadequate. Throughout, changes to the service over such a long span seem underappreciated (not least, for example, Nerva's removal of the burden from Italy).Adams modestly characterizes his contribution "'There and Back Again': Getting around in Roman Egypt" (138-66) as descriptive rather than analytical. His aim is to demonstrate the scope and value of the testimony preserved on papyri, and he does this engagingly, with unfailing regard for the impact of Egypt's unique geography and topography on travel there. Even though its publication dates back to 1952, few readers are likely to be familiar with the extensive archive preserving an itinerary and accounts for Theophanes' return journey from Antinoopolis to Antioch around 320 (160-62). Even fewer will be aware of the record of travelers and animals accommodated overnight, and rations issued to them, kept by the two large, busy mansiones in the Oxyrhynchite nome during the mid-fourth century and only published in 1994 (142-44). Vivid illustration of the multiple hazards awaiting travelers by road from causes both natural and human (154-56) can serve as a sobering counterbalance to the ideal conditions evoked by Aelius Aristides and quoted earlier by Adams (as eulogy) in the Introduction (1-2). One manifestation of "recreational" travel, which seems strangely absent from the discussion, on page 157 in particular, is pilgrimage (note, in general, Der Neue Pauly, s.v. Pilgerschaft 1).Jon Coulston's "Transport and Travel on the Column of Trajan" (106-37) is another predominantly descriptive contribution but one well geared to substantiate its conclusion: "The conflict is not simply between Romans and Dacians, Trajan and Decebalus. It is also a contest between Romans and barbarians, disciplina and perturbatio, labor and inertia, imperial virtues and enemy perfidy, civilisation and untamed nature, urbanisation and wilderness" (129-30). Description, moreover, is justified here when no other piece of Roman propaganda art, [End Page 530] before or after, depicts armies on the move—tents (for which soldiers were charged! [Tacitus Annals 1.17.4]), carts, pack animals, equipment, provisions, along roads, over bridges—with such amazing fullness and detail. We possess no more forceful illustration of the huge "train" that had to accompany every legion (118-19; the comparison made with Ottoman armies on campaign in the sixteenth and seventeenth centuries is instructive). This article attempts a contextual study of the origin and early development of August Kekulé's theory of aromatic compounds. The terminus a quo is essentially August Hofmann's coining of the modern chemical denotation of ‘aromatic’ in 1855; the terminus ad quem is the first full codification of Kekulé's theory in the sixth fascicle of his Lehrbuch der organischen Chemie, published in the summer of 1866. Kekulé's theory is viewed in context with the earlier and concurrent experimental work of such chemists (...) as Hermann Kolbe, Friedrich Beilstein, Rudolph Fittig, and Hugo Müller. The reception of the theory is briefly examined. Attention is paid to the role of Kekulé's molecular models and of his celebrated dream anecdote of the snake that seizes its own tail. The episode is used as a case study for the continuity of scientific progress, and to illustrate the close reciprocal interactions of hypothesis and experiment in the evolution of a scientific theory. Kolb's purpose is to dispel a practical and theoretical illusion--promoted by modernity about its own uniqueness as a unified and unifying constellation of meanings which wholly defines us--and to put modernity "in its place" within that sustaining and limiting context which makes it possible, which cannot be described in "standard modern terms," and which suggests that we are not as free as--and yet more than what--modernity says we are. He begins with an examination of the "standard self-description of modernity (...) and its basic dichotomies and divisions," in particular the modern preference for formal over substantive rationality, emphasized by Max Weber in his presentation of modern society as "a process waiting for content to come from the choice of its citizens"--the modern self relating "to itself and to others as a pure chooser, with no necessary substantive content". Corporate governance and finance are dynamic academic fields that offer myriad opportunities for business ethics analysis. Within the corporate governance triad in recent years, shareholders have increased their power over boards of directors and executives through both regulation and movements to change corporate by-laws. The impact of board characteristics on firm performance has proven elusive, leading to questions concerning board processes and individual director beliefs and behaviors. At the same time, CEOs have lost considerable power, leaving many struggling to regain (...) their control and maintain their compensation levels, while others adopt a stewardship approach to their posts. In the field of finance, the recent financial debacle has led to a reexamination of financial regulation and of the fundamental nature and purpose of the industry. A cross-sectional study to ascertain what the Singapore population would regard as material risk in the anaesthesia consent-taking process and identify demographic factors that predict patient preferences in medical decision-making to tailor a more patient-centered informed consent. A survey was performed involving patients 21 years old and above who attended the pre-operative evaluation clinic over a 1-month period in Singapore General Hospital. Questionnaires were administered to assess patients’ perception of material risks, by trained interviewers. Patients’ demographics were obtained. Mann–Whitney U (...) test and Kruskal-Wallis one-way analysis of variance was used. Statistical significance was taken at p < 0.05. Four hundred fourteen patients were eligible of which 26 refused to participate and 24 were excluded due to language barrier. 364 patients were recruited. A higher level of education, being employed and younger age group are factors identified in patients who wanted greater participation in medical decisions. Gender, marital status, type of surgery, and previous surgical history did not affect their level of participation. The complications most patients knew about were Nausea, Drowsiness and Surgical Wound Pain. Patients ranked Heart Attack, Death and Stroke as the most significant risks that they wanted to be informed about in greater detail. Most patients wanted to make a joint decision with the anaesthetist, instead of letting the doctor decide or deciding for themselves. Discussion with the anaesthetist is the preferred medium of communication compared to reading a pamphlet or watching a video. Age and educational level can influence medical decision-making. Despite the digital age, most patients still prefer a clinic consult instead of audio-visual multimedia for pre-operative anaesthetic counselling. The local population appears to place greater importance on rare but serious complications compared to common complications. This illustrates the need to contextualize information provided during informed consent to strengthen the doctor-patient relationship. (shrink)

At some distance from a high-rate source in an elastic half-space, the dominant wave motion at the free surface is a Rayleigh surface wave. The calculation of surface waves generated by a concentrated force in a half-space is a basic problem in elastodynamics. By straightforward manipulations, the result can be used to obtain surface waves for other kinds of wave-generating body-force arrangements. For example, appropriate combinations of double-forces (or dipoles) can be used to represent the surface loading due to laser (...) irradiation, or due to acoustic emission from the opening of a sub-surface crack or from sliding over a fault surface. The surface wave motion is usually obtained by the application of integral transform techniques and the subsequent extraction of the surface waves as the contributions from poles in the integral for the inverse transform. In this paper, we use a much simpler approach based on the elastodynamic reciprocity theorem. We consider a transversely isotropic solid whose axis of symmetry is normal to the surface of the half-space. Surface wave pulses generated by a single force, by irradiation from a laser source, and by opening of a crack, have been determined. (shrink)

J. Arthur Baird is the author of several important books in New Testament studies, his best known perhaps being his Audience Criticism and the Historical Jesus. At his untimely death, he left a nearly complete manuscript, now published here. In this timely and relevant manuscript, Baird offers first a critical introduction to the historical paradigm, pointing out its limitations in terms of tracing the paradigm of New Testament formation. He then traces this development himself, beginning with the starting point of (...) the Holy Word and noting its development through the context, theological interpretation, and application, to its manifestation in the apostles, schools and fathers. He concludes with comments on its current implications. This book represents the last and lasting final thoughts of a scholar who was very much devoted to the importance of the Holy Word. (shrink)

J.S. Mill's plural voting proposal in Considerations on Representative Government presents political theorists with a puzzle: the elitist proposal that some individuals deserve a greater voice than others seems at odds with Mill's repeated arguments for the value of full participation in government. This essay looks at Mill's arguments for plural voting, arguing that, far from being motivated solely by elitism, Mill's account is actually driven by a commitment to both competence and participation. It goes on to argue that, for (...) Mill, much of the value of political participation lies in its unique ability to educate the participants. That ability to educate is not, however, a product of participation alone; rather, for Mill, the true educative benefits of participation obtain only when competence and participation work together in the political sphere. Plural voting, then, is a mechanism for allowing Mill to take advantage of the educative benefits that arise from the intersection of competence and participation. (shrink)

Several studies describe intersectoral collaboration in Western healthcare as hampered by lack of coordination of care and treatment and incoherent patient pathways. We performed an ethnographic study following elderly patients from admission to an emergency unit (EMU) to discharge and further treatment and care at other facilities in the healthcare system. The aim was to explore how health professionals work together across sectors in the Danish healthcare system and how they create patient pathways for elderly patients (+65) with multiple chronic (...) illnesses. Intersectoral collaboration was identified as distant relations between large numbers of health professionals, where communication was conveyed by electronic health record (EHR) formats which promoted information delivery that focused on patients’ immediate symptoms. Other significant ‘mediators’ of communication were the telephone that seemed to resemble face‐to‐face communication and the patient who delivered information from one professional to another. We suggest that the communication among professionals at various facilities interacts with the format and functionalities of the EHRs, which typically fall short in delivery of information across sectors, because the often complex needs of patients with multimorbidity do not fit in with the available functionalities of the EHR.
